Patient Information Form

We have made available for download our Patient Information Form and New Patient History Form. Please click on the links to download, then print them out, fill it in, and bring it with you to your first appointment.

This helps us understand your health history. It is also kept confidential according to our privacy policies, and will remain a part of your BOSS permanent medical record.

Try to be as complete as possible, as this form is the starting point for evaluating your orthopaedic needs. We appreciate your honesty and thoroughness.

HOSPITALS

Our providers work at local hospitals to provide you with the finest possible care in the most convenient setting. Modern techniques and equipment permit these smaller hospitals to provide a high quality setting for our physicians so they may perform many surgical procedures in a location that is more convenient and closer to our patients' homes.

We provide on-call services for emergencies 7 days a week, 365 days a year. If you are hospitalized for orthopaedic care, you will be visited each day by your physician. You will also be advised prior to discharge from the hospital as to your home medications and activity level. The doctor will be happy to answer all your questions and will instruct you as to when you need to schedule a post-operative appointment.

PRESCRIPTIONS

Except in emergencies, please direct calls for prescriptions to the office from 9am to 3 PM, Monday through Friday. Please have the name of the pharmacy and their telephone number, name and strength of the medication, the dosage schedule, and the prescription number when you call. Prescriptions for narcotic pain medication will not be renewed outside of office hours, or on the weekend because the physician must review the patient's medical chart.
