FEES

Fees charged for professional services are reviewed regularly; if you have any questions or concerns, please direct them to the business office. A specified  period of post-operative care will be included in the charge for surgical procedures. This includes routine post-operative treatment,  but not unexpected problems or unrelated conditions that may arise. The fracture care charges include follow-up office visits sufficient for  the care of the specific fracture, but do not include x-rays, casts, splints, or other orthopaedic equipment that may be required.

BILLING AND INSURANCE

We recognize the need for a definite understanding between the patient and his/her doctor regarding financial arrangements for medical and surgical care. For those patients receiving financial benefits from insurance or government agencies, we are required by law to collect the appropriate co-pay and deductibles specific to your agreement with the agency involved. The responsibility for payment of these fees is the obligation of the patient. Because of the high cost of statements and billing, we request payment at the time of service, unless otherwise included as part of your surgical or fracture charge.

If  you do not have medical insurance, an itemized statement will be provided and payment is expected at the time of service. A payment plan  may be worked out if necessary. All telephone calls concerning billing should be directed to the Insurance Department by asking the operator to connect you.

INSURANCE INFORMATION

The  patient registration coordinator will ask you to complete an information form. As a service to our patients, we file your insurance claims for you. Therefore, the questions on the form are necessary in order to provide you with the best service possible. We ask that you update any changes in this information to insure proper claims submission. As with your medical information, all insurance information  you provide is strictly confidential.
